Step 2) FILL OUT THE CORRECT FORM AT THE BOTTOM OF THIS PAGE. (a secure copy will go to your email). It’s that easy.

You may book a free consultation to learn more about any service. There is no need to print the forms. Did you fill out the online form in the past year? If so, no need to do again.

Step1: Book Your Appointment

You will need a valid credit card to hold your spot. Payments are taken at time of service or prepay if desired. You may change or cancel appointments in your email confirmation up to 24 hours in advance. No-shows will result in a $30 charge and prepayment in the future. If there is no spot to meet your schedule, call us directly at 800-410-1678 to fit you in (even right away). If your service was prepaid, please select a prepaid option.

• MEDICAL CARE FORM: For Shock Wave Treatment, Vanquish ME Belly Fat Reduction, Erectile Dysfunction, Concierge Medicine, Weight Loss, Lab Tests, Nutraceutical Plans, Hormone Balance, Diabetes, Hypertension, High Cholesterol, Colds, and Everything Else.

• TELEMEDICINE FORM: For video or phone consultations.

• B0T0X OR FILLER FORM: For B0T0X and Fillers

• PLASMA PEN OR MICRONEEDLING FORM: For Hydrafacials, Microneedling, Pshot, Oshot, Joint Injections, Scalp Injections.

• CONSENT FORM FOR IV VITAMIN, MINERAL AND ANTIOXIDANT THERAPY: Consent form for IV infusions with vitamins, electrolytes, glutathione, NAD, and other options.
